Subscription Services Cancellation and Refund Policy Last Updated: March 30, 2026

Subscription Services Your membership in the subscription program ("Subscription Service"), including your ability to access the services, begins when you have completed the intake form at which time you will be charged for the first month of the Subscription Services.

The Subscription Services include access to providers, medications where appropriate, and online support.

Please note if you are disqualified by our medical provider for any medical reason you will receive a full refund.

There is no guarantee of results or effectiveness due to your participation in the Subscription Services.

All medical evaluations are performed by independently licensed healthcare providers affiliated with OpenLoop Healthcare Partners, PC and its affiliated professional entities (the "Practice"). Kora Health, LLC ("Kora MD") is a technology platform and administrative services provider.

Kora MD does not practice medicine, does not prescribe medications, and does not guarantee that you will receive any specific medication.

The decision to prescribe medication is made solely by the treating healthcare provider in the exercise of independent medical judgment.

If a prescription is issued and medication is fulfilled, prescription medications are dispensed by one of the following independently licensed pharmacy partners: RedRock Pharmacy (1240 E 100 S #220, St. George, UT 84790; 801-433-9500; https://www.redrockhomepharmacy.com/); Health Warehouse (7107 Industrial Rd., Florence, KY 41042; 800-748-7001; https://www.healthwarehouse.com); Precision Compounding Pharmacy (2657 Merrick Road, Bellmore, NY 11710; 516-833-6262; https://mypcphealth.com/); and Triad Rx (26258 Pollard Road, Daphne, AL 36526; 251-380-7630; https://www.triadrx.us).

Cancellation Policy You may cancel your Subscription Service at any time for any reason.

To cancel your Subscription Services, please contact customer service at care@koramd.com, call (855) 597-1248, request a cancellation through the process outlined in your online account (if applicable), or email patientsupport@openloophealth.com.

The cancellation process is designed to be at least as simple as the method you used to subscribe.

To ensure a smooth process, we require your cancellation request to be received at least 72 hours prior to your billing date.

If this notice date is not met, your subscription will be charged on the next billing date, and the cancellation will take effect on the subsequent billing date.

After cancellation, you will continue to receive the Subscription Services through the end of your billing cycle.

Kora MD does not charge cancellation fees, early termination fees, or penalties for canceling a Subscription Service.

Kora MD will send you a confirmation of cancellation via email within a commercially reasonable time following receipt of your cancellation request.

Please see our Refund Policy below to determine if you are eligible for a refund of your canceled Subscription Service.

Refund Policy If your Subscription Service is cancelled due to disqualification by our medical provider, you will be issued a refund for the remainder of your Subscription Service charges following the disqualification.

Other than cancellation due to medical disqualification, IN NO EVENT SHALL YOU BE ISSUED A REFUND UPON CANCELLATION OF THE SUBSCRIPTION SERVICES.

Federal law generally prohibits the return of prescription medications to pharmacies for refund or reuse once they have been ordered to a patient in order to protect public health and safety.

This policy ensures that all medications provided are handled and dispensed with utmost care and compliance.

If approved, refunds will only be issued for your most recent billing cycle and will not be issued for past billing cycles.

Damaged or Incorrect Items: Please inspect your medication immediately upon receipt.

If the medication appears to be damaged or incorrect, please contact customer service immediately at care@koramd.com.

Damaged items: Damaged items may be replaced by the pharmacy upon receipt of evidence of damage.

Incorrect items: Incorrect medications will be replaced by the pharmacy.

All approved refunds will be issued to the original payment method within ten (10) to fifteen (15) business days of approval.

Kora MD is not responsible for delays caused by your financial institution in processing the refund.

Subscription Services FAQs What is included in my Subscription Services price? Your Subscription Services represents a single price for the subscription purchased.

This may include consultations both live and via electronic form submissions and reviews with a Provider for medications that are fulfilled through a pharmacy or other health care services on which we collect payments, lab services, care support, patient support line and other services to support your medical weight loss journey.

Services, labs and medications covered with the single Subscription Service price may vary.

All medical services are provided by independently licensed healthcare providers affiliated with the Practice, and all medications are dispensed by the independently licensed Pharmacy Partners listed above.

What if my provider determines I am no longer eligible for the Subscription Services? If your Provider determines you are no longer eligible for the Subscription Services, your subscription will be canceled, and a refund may be issued for the remainder of your current billing cycle.

How do I cancel my Subscription Service? You may cancel your Subscription Service at any time through any of the following methods: (1) by accessing the cancellation function within your online account settings (if applicable); (2) by sending an email to care@koramd.com with the subject line "Cancellation Request" and including your full name and the email address associated with your account; (3) by calling March 30, 2026 during business hours; or (4) by emailing patientsupport@openloophealth.com with your name and email address associated with the service.

What happens after I cancel? After cancellation, you will continue to receive the Subscription Services through the end of your current billing cycle.

You will not be charged for any subsequent billing cycle following a timely cancellation.

Kora MD will send you a confirmation of cancellation via email.

Governing Terms Unless otherwise stated herein, the Kora MD Terms and Conditions govern the Subscription Services Cancellation and Refund Policy.

In the event of any conflict between this Cancellation and Refund Policy and the Terms and Conditions, this Cancellation and Refund Policy shall control with respect to cancellation and refund matters.

In all events you are responsible for all fees from services and Healthcare Services rendered.

This Subscription Services Cancellation and Refund Policy governs fees for Subscription Services only.

Service Availability Currently offering services in all 50 states plus Washington D.C.

Some services may not be available in all 50 states or Washington D.C.

Part 425, as amended 2025), which requires that cancellation of recurring subscriptions be at least as easy as the method used to subscribe.

All cancellation methods described above satisfy this requirement.

Prorated Refunds for Product Unavailability: If Kora MD, through its Pharmacy Partners, cannot fulfill a treatment plan due to supply shortages, FDA regulatory changes, or other factors outside its control, Members are eligible for a prorated refund calculated based on the unshipped portion of their prepaid plan, with deductions only for the monthly cost of treatments already provided.
